Medicare Supplement Plans North Carolina 2027: What 55 Plan G Prices Tell You

Reviewed by Russell Noga, Licensed Medicare Insurance Broker (all 50 states) — Last updated October 29, 2026

Medicare Supplement plans in North Carolina for a 65-year-old man start around $111 a month for Plan G and run past $266 a month for the same coverage, based on the NC Department of Insurance premium database in October 2026. North Carolina has no birthday rule, and nearly every NC plan raises its price as you age, so the company you pick at 65 matters for years.

North Carolina is one of the few states that publishes every Medicare Supplement company’s price in a public database. Its Department of Insurance and its SHIIP counseling program let anyone look up monthly premiums by plan letter, age, sex and tobacco use. We pulled the Plan G list for a 65-year-old non-smoking man in October 2026 and did the math so you do not have to.

The short version: identical Plan G coverage can cost more than twice as much depending on the company, and in North Carolina the starting price is only half the story. If you are new to the plan letters, our Medicare Supplement plans 2027 guide explains each one.

Source: NC Department of Insurance Medicare Supplement Premium Comparison Database, Plan G, man 65, non-tobacco, viewed October 2026. Some companies list more than one policy. Rates may not include discounts, and your ZIP code can change the price.

Nearly half of the listed prices sit under $130, so paying $200 or more for a new Plan G at 65 usually means you did not compare. Women, smokers and older applicants will see different numbers.

The Catch: Most NC Plans Raise Prices as You Age

The database sorts plans by how they are priced. Of the 55 Plan G prices we counted, 51 were attained-age policies, 2 were issue-age and 2 had no age rating. North Carolina lets each company choose its method, and nearly all choose attained age.

Attained age. The price is based on your current age, so it goes up as you get older, on top of any yearly rate increases.

Issue age. The price is based on your age when you bought the policy. It still rises with inflation, but not because you had a birthday.

No age rating. Everyone pays the same price regardless of age, so it often looks expensive at 65 and cheaper later.

Broker tip: Because North Carolina has no birthday rule, a low attained-age price at 65 is worth checking for its rate history. Once you have health problems, moving to another company can mean answering health questions.

No Birthday Rule in North Carolina: When You Can Switch

Medicare Supplement plans North Carolina switching rules: open enrollment, guaranteed issue and no birthday rule

North Carolina does not have a yearly birthday or anniversary rule. That makes your first window the one to get right.

1

Your one-time open enrollment. Six months that start the first month you are 65 or older and enrolled in Part B. No company can turn you down or charge more for your health.

2

Federal guaranteed issue events. Losing employer coverage, a plan leaving your area or leaving Medicare Advantage during a trial period can give you a short window.

3

Every other time. You can apply whenever you want, but the company can review your health and decline you.

Waiting periods: Even during open enrollment, a NC Medigap policy can make you wait up to six months for a pre-existing condition, unless you had prior creditable coverage.

Under 65 on Medicare in North Carolina: Plans A, D and G

Federal law does not guarantee Medigap to people under 65, but North Carolina law does. Under G.S. 58-54-45, effective January 1, 2020, people under 65 who qualify for Medicare can buy Plan A, D or G.

Plan A for everyone. Every company must offer at least Plan A to people under 65 on Medicare.

D and G if sold at 65. A company that sells Plan D or Plan G to people 65 and older must offer it to eligible people under 65 too.

Higher prices are allowed. The NC Department of Insurance warns that premiums may be higher for people on Medicare due to disability.

A second chance at 65. When you turn 65 you get a new six-month open enrollment and can buy any plan at regular age-65 prices.

No. North Carolina does not give a yearly birthday or anniversary window. Outside your first open enrollment and federal guaranteed issue events, companies can ask health questions.

Likely. Companies file rate changes during the year, and higher 2027 Medicare deductibles can push plan costs up. Check current prices before you buy.
